Benefits of PLDD

PLDD (Percutaneous Laser Disc Decompression) offers several advantages in appropriately selected cases.

Local anesthesia: the procedure is performed under local anesthesia (not to be confused with spinal anesthesia, where the patient remains awake but the entire lower body is numbed). This can be particularly helpful for patients with comorbidities that may increase the risks associated with general or spinal anesthesia.

May also be suitable for some disc protrusions: in addition to a broad group of disc herniations (not all—this is important), PLDD can also be used for disc protrusions, which are often small yet can be very painful. In these situations, a “classic” or endoscopic surgical indication is usually not appropriate. PLDD may be considered when conservative treatments (medications and/or physical therapy, always the first-line approach) have not provided relief and symptoms significantly affect daily life and work.

Recurrent herniation after surgery: in a subset of cases, PLDD may also be used for recurrent disc herniations after open surgery or an endoscopic procedure (a not uncommon situation). However, success rates are typically lower in these cases (approximately from 85% to 65–70%).

Faster recovery: PLDD often allows a relatively quicker return to daily and work activities (typically within 3–4 weeks), with potentially meaningful socio-economic benefits. In fact, in about 70% of cases, disc herniations and protrusions affect people of working age.

Minimally invasive: no surgical incision is required. A very thin needle is used to pass through the skin, subcutaneous tissue, and muscles and—under radiologic guidance—reach the center of the disc (the nucleus pulposus). Because there is no incision, no muscle detachment from the vertebral laminae, and no removal of bone or ligaments, the risk of certain issues seen with traditional surgery is reduced, such as periradicular scarring (adhesions that may involve and compress nerve roots), chronic pain related to muscle injury, and sometimes spinal instability that could require more complex re-operations.
